Innovative development of new AI blood detection technology for liver cancer detection

The blood test is called DELFI, short for Early Interception DNA Fragment Assessment, and it detects changes in a segment of cancer cell DNA, known as cell-free DNA, or cfDNA, that flows into the bloodstream. In the latest study, researchers used DELFI technology to examine plasma samples from 724 people in the United States, the European Union and Hong Kong to detect hepatocellular carcinoma (HCC), a type of liver cancer.

The researchers believe this is the first genome-wide fragmentation analysis independently validated in two high-risk populations, involving different causes of liver cancer associated with different races and ethnicities.

The DELFI technique uses blood tests to measure the way DNA is packed inside the nucleus by studying the size and amount of free DNA that is present in circulation in different regions throughout the genome. Healthy cells pack DNA like a well-organized suitcase, in which different regions of the genome are carefully placed in separate compartments. The cell nucleus of a cancer cell, by contrast, is like a more disorganized suitcase, into which items from the entire genome are tossed. When cancer cells die, they release pieces of DNA into the bloodstream in a chaotic fashion.

In the latest study, researchers tested cfDNA fragments isolated from plasma samples, a test that has previously been shown to accurately classify lung cancer. They analyzed the fragmentation patterns of each sample to develop a DELFI score.

Scores were lower in cancer-free individuals with viral hepatitis or cirrhosis, but among the 75 HCC patients in the US/EU sample, the average score was 5 to 10 times higher, with high scores observed in all stages of cancer, including early disease. In addition, the test detected fragmentary changes in liver cancer genome content and packaging, including genomic regions associated with liver-specific activity.
